What does a nutrition specialist/dietotherapist do and how is he/she distinguished from a nutritionist?

Short explanation of the title – According to the old law on specialisations, that was “a subspecialty in nutrition of healthy and diseased people“ and that was the very name of the certificate and the licence. According to the new law, only the name has changed and it now reads as “dietotherapy” and that is the very name of the title as well.

A dietotherapist establishes nutritional diagnosis and prescribes treatment.

Regardless of the fact whether it is an independent measure or it is applied with medicines or at the request of other specialists, a nutritional therapeutic measure which can be in the form a structured diet plan, MNT or just diet advice is ALWAYS prescribed by a dietotherapist. That is what essentially distinguishes him/her from a nutritionist (the precise term - vocational nutritionist/dietitian), which is a degree awarded upon the completion of a three-year Bachelor programme of applied studies, in which the participants to receive training on preventive and balanced diet, but which does not rain them to prescribe “diets” independently. They can be part of a team treating a patient only on condition that they are under the control of the attending physician.

What does the examination involve and what should you prepare for it?

An examination by a dietotherapist does not require any special preparation of the patient.
However, if the patient has any previous medical reports from other specialists – an internist, a surgeon, a paediatrician, etc – he/she should certainly bring them with himself/herself.

If the lab test results he/she potentially has are more than six months old or he/she does not have them at all, the following blood tests should be done prior to the examination:

  • a complete blood count
  • (CBC) with leukocyte formula
  • a serum iron test
  • a ferritin test
  • a glycemic index, HbA1C test
  • a lipid panel test
  • a hepatogram
  • a urea (BUN) test
  • A creatinine test
  • a GFR – glomerular filtration rate test
  • a uric acid test
  • a CRP test
  • a TSH test
  • a FT3 test
  • a FT4 test
  • a vitamin D test

 

The examination begins by your personal and family medical records, i.e. medical history being taken. It is followed by the nutrition examination, the analysis of the blood test results and additional medical documentation. The nutrition examination also involves the analysis of the lifestyle elements which are essential for diet and physical activity, which give each of us a strictly personal stamp.

The examination ends with the synthesis of all data obtained and nutrition diagnosis being set. That is followed by a therapy being prescribed or by the referral to other specialists, when that is necessary.

Dr. Jorga’s method

The basic ethical principle  “Primum non nocere “ (First, no harm) and the well-known medical doctrine  “Dosis sola venum facit “ (The dose makes the poison) are what guide us in everything we do, including the prescribing of medicines as well as food supplements.

The optimum intake of essential and necessary nutrients in each required calorie intake as the basic principle of the nutritional science is built into each of our recommendations. Diet models are individual and structured, and, if necessary, they are phased. There are clearly defined goals for each individual patient, which are set, monitored and changed periodically, in line with the achievement.

Dr. Jorga works with persons of all ages, starting from seven-year-olds on, regardless of the fact whether they are healthy, have already been diagnosed with a disease or consider their diet to be improper. 

Medical nutrition therapy (MNT)

You will come across this term increasingly often within the therapy prescribed by various specialists. An MNT or a diet in its true sense, the one without quotation marks, is a medical therapeutic measure. It is prescribed by the attending physician with the aim of enabling  better and more efficient treatment of a disease, regardless of the fact whether it is a single measure or it accompanies other forms of treatment.

The manner of administering nutrition therapy and support may be parenteral – intravenous, enteral – directly administered to the stomach/intestines or oral – through the mouth.  The first two forms are implemented by anaesthesiologists, surgeons, gastroenterologists, most frequently in hospitals, whereas oral nutrition therapy is most often prescribed by a dietotherapist and it is administered under any conditions, including administration at home.

What does a structured diet model involve?

it also takes into consideration your habits, social and cultural environment, dieting preferences, including fasting, veganism, food intolerance and the like.

A nutrition model also includes the list of foods – which, when, in what quantities and in what combinations they are to be taken, how they should be prepared, how many meals there should be and when you should have them, with simple examples.

For those who wish it, the service of making a detailed two-week diet plan, which is made separately and is not included in the primary service, is available.
